This form should be completed by the member of the Association (home owner), or someone representing the Association member. This form should be used for maintenence concerns and/or physical damage inspection and repair concerns.
NOTES: The form is for homeowners association members. If you are renting your space, please reach out to your landlord or property management company.
The HOA will only maintain or repair general common areas and CANNOT repair or fix anything that is limited common areas or inside of the units. This includes windows, doors and outside fixtures that would fall to unit owners to maintain and repair.
